Mechanism of Action:

Inhibition of bacterial protein synthesis

Indications:

Azithromycin is used in the treatment of respiratory tract infections, otitis media, skin and soft tissue infections, uncomplicated genital chlamydial infections, and non-gonococcal urethritis.

Dosage and Administration:

Adults: 500 mg on the first day and 250 mg on days two to five. An alternative regimen is: 500 mg once a day for 3 days

Children: For children over 6 months of age: 10 mg per kilogram of body weight on the first day and 5 mg per kilogram of body weight on days two to five once a day. An alternative regimen is: daily 10 mg per kilogram of body weight given once a day for 3 days

Take azithromycin capsules at least one hour before meals or two hours after meals.

Contraindications:

Azithromycin should not be used in cases of hypersensitivity to any macrolide antibiotics and in patients taking astemizole, cisapride, or pimozide. Azithromycin should also not be used in patients with impaired hepatic function.

Side Effects:

Mild gastrointestinal disturbances, temporary increase in liver enzymes, and rarely obstructive jaundice, rash, headache, dizziness, and hypersensitivity reactions and temporary changes in neutrophil levels.

Drug Interactions:

The serum concentration of disopyramide may increase when taken simultaneously with macrolides, potentially leading to toxicity and arrhythmia. The effect of warfarin may be increased when taken with macrolides. Macrolides inhibit the metabolism of carbamazepine, bromocriptine, theophylline, and cyclosporine, possibly leading to increased serum concentrations of these drugs. Macrolides also inhibit the metabolism of terfenadine, increasing the risk of arrhythmia. The effect of digoxin on the heart may increase when taken with macrolides. The serum concentration of macrolides is increased by cimetidine, which may enhance the risk of side effects and toxicity, especially deafness caused by macrolides. However, overall, the drug interactions of azithromycin are much less than those of erythromycin, which is considered one of the strengths of this medication.

Use in Pregnancy and Lactation:

Pregnancy Category B. This medication should be used with caution during pregnancy and breastfeeding.
